Providence is calling a Transition Planning Social Services II (0.78 FTE, Day Shift) to the Social Services at Providence Regional Medical Center Everett in Everett, WA.

We are seeking a detail-oriented licensed Social Worker to plan and coordinate activities related to transition planning. Develops and implements a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care.

In this position you will:

  • Demonstrates Mission/Values.
  • Assures individual respect for all while maintaining composure in dealing with difficult situations
  • Demonstrates age specific patient care considerations for the population served
  • Recognizes and assesses individual patient and family needs or requirements for safe and appropriate discharge from the acute care setting
  • Leads, develops and implements appropriate discharge plans for patients transitioning from an acute care facility to the next appropriate level of care or community
  • Works collaboratively with physicians, other healthcare team members, community agencies and family to develop a patient/family-focused discharge plan that identifies needed resources and linkages to achieve timely and optimal discharge

Qualifications:

Required qualifications for this position include:

  • Master’s Degree in Social Work or Counseling
  • Current Agency Affiliated Counselor, Licensed Mental Health Counselor or Associate; Licensed Advanced Social Worker or Associate; or Licensed Independent Clinical Social Worker or Associate.
  • WA State DOH licensure
  • Experience in discharge planning for medical and or surgical patients in an inpatient or outpatient setting

Preferred qualifications for this position include:

  • Experience in an acute hospital setting doing discharge planning
