We are seeking LVN’ s interested in Shift Care to provide skilled nursing to patients within their home. Our ideal candidate must be compliant and punctual with shifts and Electronic charting . Must be Bilingual, Good communication skills with staff and multidisciplinary team is required. Candidates must have a reliable method of transportation and have open availability for shifts of interest.
HIRING FOR: (The shift availability is open Monday through Sunday, although it is not a requirement to work all those days. Scheduling can be discussed upon interview.) (These are 6 separate shifts in Rialto)
1. Monday through Sunday (8hr shifts) – 10AM – 6PM
2. Monday through Sunday (8hr shifts) – 10AM – 6PM (Spanish Speaking Only)
3. Friday (10hr shift) – 5AM – 3PM, Saturday & Sunday (8hr shifts) – 5AM – 1PM
4. 3 Days a week (12hr shifts) – 7AM – 7PM
5. Saturday (8hr shift) NOC – 10PM – 6AM
6. Monday (8hr shift) – 10am – 6pm
Most of our patients require (2) or more of the following Nursing Skills
**If not familiar with these skills, training will be provided***

- Ventilator
- Tracheostomy
- Suction
- Gastrostomy
- Seizure Precautions
- Oral, SubQ, IM medication administration
Rate: $26-28/hr
Credentials Required:
- Valid professional licensed in California
- Personal Identification Card (ID)

- Permanent Resident card, or work permit
- Physical exam
- Tuberculosis Results have to be within a year or negative results upon hire
