The Quality of Life Coordinator is responsible for planning, scheduling and coordinating meaningful activities appropriate to the needs and interests of dementia/Alzheimer?s and other resident populations. The activities will be conducive to their physical, mental and social well-being. The Quality of Life Coordinator will function as part of the interdisciplinary team and also be able to work independently. The Quality of Life Coordinator reports to the Executive Director.
Job Duties and Responsibilities
- Practice MOSAIC quality of life program to enhance residents mind, body and spirit
- Encourage, mentor and empower staff to consistently practice MOSAIC program
- Ensure activity programs are appropriately presented to and by to staff
- Plan, schedule and coordinate resident activities on a monthly basis, with staff input
- Provide program content materials for activities
- Plan or coordinate quarterly family parties/events
- Plan and coordinate arts and crafts program, entertainment for special events, monthly birthday parties, group games, weekly religious services and individual independent projects that reflect the MOSAIC Program
- Provide community integration through recreational activities from the community and develop close working relationships with community organizations. Also provide volunteer activities to bring the community into the house
- Maintain adequate supply of books, arts and craft materials, puzzles, magazine, games and other items in the sensory cart and on the standard activity list and house
- Team member orientation, training in activities and Resident Activity Profile
- May assist the Director in performing in performing various administrative functions including but not limited to:
- Admission paper work
- Team in-service programs
- Coordinate and facilitate non-emergency transfers bi-annual input for ISP?s
- Conduct building tours
- All other duties as may be assigned
