Harvard Family Physicians is a multi-specialty primary care clinic with 21 providers in Family Medicine, Pediatrics and Obstetrics & Gynecology.

Job Summary:

Patient Care Managers are responsible for the following:

• Pre-Visit planning
• Preventative services
• Care Management of High-Risk Patients
• Admit/Discharge transfer calls
• Longitudinal Care with plans of care on high risk patients
• Patient Education on Chronic conditions

Job Type: Full-time

Day Shift Only (Monday – Friday 8:00 am to 5:00 pm)

Pay: $24.00 – $32.00 per hour
