Medallion Hospice – Easterseals-Goodwill Northern Rocky Mountain – Great Falls, MT

Assists in coordination of in-home support services to meet the needs of Hospice patients and their families. Performs introductory visits and admissions. Makes initial and ongoing evaluation of the patient and family’s needs, including need for change in medical management to control symptoms and maintain comfort. Will be responsible for providing nursing services consistent with high standards of performance and conduct with currently available knowledge and nursing skills. Applies diagnostic and crisis intervention skills. Coordinates resources available to meet patient and family’s needs. Reviews all processes on a yearly basis to assure those processes are current and within or exceeding guidelines. Assists with and completes other tasks as requested. After hours and weekend on-call required.

Required Skills

  • Adheres to nursing conduct standards as defined by the State of Montana.
  • Completes introductory visits to explain hospice services to patients and families.
  • Works with Quality and Compliance Manager, physician, and Clinical Manager to ensure each referral is evaluated prior to admission to determine appropriateness, evaluate source of payment, and ability to meet health and safety needs.
  • Completes admission process for patients to hospice including initial assessment, certification, developing plan of care and obtaining required physician approval/signatures.
  • Responsible for timely completion of recertification regarding the patients’ eligibility for hospice.
  • Follows all hospice protocols, policies and procedures carefully.
  • Carefully follows procedure at time of death and offers appropriate support to the survivors, ensuring all appropriate team members are notified of the death.
  • Provides nursing care to patients in their home setting, under the direction of primary physician.
  • Responsible for providing coordination of care at Interdisciplinary Team Meetings.
  • Regularly assess and evaluate patient’s condition while providing education and support to patient and family.
  • Performs prescribed medical treatment, including pain management and symptom control, under direction of primary physician or Hospice Medical Director.
  • Notifies the primary hospice nurse and applicable team members of any patient who has experienced significant distress, change in status or death during the work shift.
  • Maintains a current and complete record in order to have pertinent background information on each hospice patient.
  • Adjusts constructively and flexibly to meet the requirements of different work situations, including after hour and weekend on-call coverage as needed.
  • Provides supervision of hospice aides and LPN.
