Registered Nurse – Azura Memory Care – Eau Claire, WI

Mission:

The Registered Nurse is responsible for providing professional and quality person centered care for residents with Alzheimer?s disease, other types of dementia and residents with other medical or nursing needs to promote their highest well-being.

Job Duties and Responsibilities:

Clinical:

  • Practice a holistic approach to meet residents? needs and to promote their highest well-being.
  • Assess residents prior to admission, when able.
  • Visit and reassess current residents who may be in acute care or another long term care setting.
  • Utilize nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of the resident?s care, as evidenced by staff observation.
    • Assess residents: collect and document the resident?s current health status and medical history. Complete and/or review resident care plan as defined by company policy and procedure. Make accurate and ongoing assessment of resident status and respond appropriately.
    • Plan/implement: set priorities for nursing action in a logical sequence according to resident?s needs and assists ED/designee to formulate a plan of care by the appropriate selection of Nurse Practice Standards. The resident care plan is initiated on resident admission based on all aspects of the initial assessment. Plan includes physiological, psychosocial and environmental factors.
    • Evaluate: perform ongoing assessment and assists ED/designee to revise plan of care based on new data and resident?s condition.
    • Document accurate and ongoing assessment of residents? status. Document resident care which reflects nursing intervention, resident response to care provided, patient needs, problems, capabilities, limitations, and progress toward goals. Documentation may include evidence of appropriate resident/family teaching and the understanding of these instructions in the medical record.
    • Weekly after hours call and weekend/holiday call rotations as assigned.
  • Communicates with primary or alternate physician regarding residents? needs, the nursing assessments and recommendations as needed, as evidenced by Executive Director and team member observation
  • Collaborate with Director of Clinical Services & Compliance, Executive Director and team members to ensure the residents? well being and interdisciplinary approach to resident care.
  • Delegates medication administration and any other task that requires RN training
  • Monitors medication processes in assigned homes.
  • Oversees the medication management of residents by working closely with pharmacy and team members to ensure that medication orders are accurate and medications are available as ordered by the medical providers.
    • Communicates and follows up with resident POAs and Primary Care Physicians on Med changes

Administrative:

  • Monitoring of electronic charting, including observations, incident and fall reports. Follow-up emails to Director of Clinical Services & Compliance, Regional Director of Operations, and Executive Director on any incomplete reports or any serious clinical concern regarding a resident.
  • Monthly monitoring of PRN psychotropic medications and quarterly monitoring of routine psychotropic medications.
  • Participates in auditing systems as appropriate and in any regulatory process as directed.
  • Completes QIA report in Point Click Care monthly for assigned homes.
  • Participates in Safety Committee when available.
  • Participates in monthly team meetings
  • Attends quarterly company operational meetings as well as other meetings as called by the Regional Director of Clinical Services & Compliance or other members of the Leadership team. These meetings may include approved overnight travel.

Training:

  • Conducts state approved Medication, Standard Precautions, and First Aid Training, once an approved trainer, as needed.
  • Provides education and training to team members monthly or as needed to ensure best clinical practice is being performed.
  • Provides guidance to Executive Director with clinical processes.

All other duties as assigned.

Education:

  • Bachelor or associate degree in nursing preferred
  • Current, unencumbered active license to practice as an RN in the state of Wisconsin or qualifications to obtain RN license in Wisconsin prior to first day of employment
  • One year of Long Term Care experience preferred
