Medical Social Worker, Montgomery and Philadelphia County – Social Work p.r.n. – Fort Washington, PA

Medical Social Worker needed for part-time, temporary position in Montgomery and Philadelphia Counties.

Interview patients, families, significant others, evaluating their present situation, providing support, and counseling, accordingly, referring to appropriate resources. Monitors LOS (length of stay) of patient to assist in timely discharge planning.

Conduct psychosocial assessments and provides crisis intervention, emotional support and counseling services to patients. Collaborates with all member of the healthcare team, patient and/or family to develop and implement a patient centered plan of care. Obtains, interprets and communicates necessary information regarding patients and families, including social history information to the healthcare team.

Refers patient/family to hospital and community-based services to meet patient continuum of care needs. Such interventions may include referral for financial psychiatric, rehabilitative, inpatient hospice, drug and alcohol treatment, support groups. Documents the initial evaluation, as well as ongoing activity when a significant event occurs. Interacts with co-workers, visitors and other staff consistent with the core values of the hospital.

MSW with two years’ experience in a hospital setting.
