What You Will Do

Work with a multidisciplinary team to plan, organize and direct skilled home health care services, while empowering patients toward independence at home.
As a Registered Nurse (RN) with Stonerise Home Health, your primary responsibilities include:

  • Complete health assessments and develop individualized patient care plans in coordination with patient, family and multidisciplinary care team.
  • Initiate appropriate preventive and rehabilitative nursing procedures and oversee coordination of the care plan.
  • Administer medications and treatment as prescribed by the physician.
  • Supervise and direct the Licensed Practical Nurse and/or the Home Health Aide in implementation of the patient care plan.
  • Regularly reevaluate patient nursing needs and make necessary revisions as patient status and needs change.
  • Develop patient teaching plans, as needed, and direct the Licensed Practical Nurse in implementation of the teaching plan.
  • Creates and modifies the Home Health Aide care plan and supervises the home health aide via patient feedback and onsite interaction.
  • Provide health care instructions to the patient as appropriate per assessment and plan of care.
  • Regularly communicate with the physician regarding patients’ needs and report any changes in patients’ conditions. Obtain physician’s orders as required.
  • Participate in discharge planning process.

Education, Licensure & Certification Requirements

  • Nursing degree from an accredited university, current professional licensure with one year of home health, emergency department or hospice care experience preferred.
