Hospital Liaison (Community Health Center) – MHC Healthcare – Marana, AZ

MHC Healthcare is seeking a Hospital Liaison to join our Behavioral Health team at the MHC Counseling and Wellness Center, located in the heart of Marana, AZ. The Hospital Liaison (HL) ensures that discharge planning is discussed and reviewed at each hospital contact. He/she obtains and documents written or verbal agreements with the plan from the attending inpatient and outpatient Behavioral Health Medical Provider (BHMP). The HL attends hospital clinical rounds in person or by telephone, when permitted by the inpatient facility, and coordinates with the inpatient and outpatient BHMP to ensure agreement with services provided upon discharge. The HL ensures all documentation submitted is clinically appropriate and received timely as to avoid delaying discharge of any member. The HL works closely with the assigned clinical and Utilization Management Team to ensure continuity of care and timely discharge. MHC Healthcare is a Federally Qualified Community Health Center (FQHC), with 16 sites in Tucson and Pima County. The ideal candidate will have the following education and experience:

  • Bachelor’s Degree or 4 years of Behavioral Health (BH) experience; required.
  • High school diploma or GED; required.
  • Experience in BH services or court setting with at-risk youth or adults; required.
  • Experience with substance abuse treatment and case management; required.
  • Experience with the Regional Behavioral Health Authority (RBHA) system and/or community resources and agencies; preferred.
  • Must provide current 39-month Motor Vehicle Record; required.
  • Basic Life Support (BLS) and First Aid certified; required.
  • Fingerprint Clearance Card through the Arizona Department of Public Safety (DPS); required.

The ideal candidate will also possess the following knowledge, skills, and abilities:

  • Demonstrated competencies in dealing with all age groups, including neonates, infants, children, adolescents, adults and geriatric patients.
  • Applied knowledge of computer applications in a Windows-based environment.
  • Ability to coordinate and collaborate between inpatient facilities and outpatient providers to promote effective transition to aftercare/outpatient care.
  • Bilingual preferred and may be required based on patient needs.
  • Knowledge of patient appointment scheduling.

Duties and Responsibilities:

  • Responsible for coordination of client admissions and discharges from BH inpatient facilities.
  • Ensures discharge planning is discussed with the inpatient and outpatient team within 24 hours of admission and that all required documentation is completed accurately and thoroughly
  • Obtains and documents written or verbal agreements with the discharge plan from the attending inpatient and outpatient BHMPs.
  • Communicates all BH inpatient facility admissions, discharges and any significant changes to the Utilization Management Team.
  • Attends hospital clinical rounds in person or by telephone at least twice per work week, when permitted by the inpatient facility, and reviews discharge planning with the inpatient and outpatient BHMP so there is ample opportunity to collaborate on a viable discharge plan on which all agree.
  • If not available to attend clinical rounds, the HL contacts the inpatient social worker, discharge planner or BHMP on that day to discuss the discharge planning.
  • Participates in CFT or ART meetings as needed.
  • Maintains general behavioral health and education standards, keeping abreast of behavioral health and State RBHA guidelines, by reading and attending in-services and workshops as appropriate and indicated in the staff development plan.
  • Works with the Substance Abuse Supervisor to ensure all documentation submitted is clinically appropriate and submitted timely so as not to delay discharge of any member.
  • Follows professional and ethical practices when working with clients, client records, families and community personnel.
  • Maintains confidentiality of client and family information.
  • Follows incident reporting protocol and procedures when required.
  • Interacts effectively with MHC staff members, community members and other agency personnel to promote trust, productivity and growth.
  • Provides a responsible, productive and creative role model for other MHC staff.
  • Performs appropriate follow-through, from start to finish, with all assigned tasks.
  • Maintains a positive attitude towards job responsibilities, working collaboratively with all MHC staff members to create a productive working team.
  • Completes other tasks as requested.
