Arbor QMAP Assisted Living Aide (Part-time Days) – Eben Ezer Lutheran Care – Brush, CO

Eben Ezer Lutheran Care Center is seeking a motivated individual in the Arbor Assisted Living Department (AL) to provide cleaning, personal care for our neighbors in their apartments and a safe environment for our neighbors to live in. We value individuals who can build trust and rapport with the neighbors.

The qualified candidate must:

  • Be at least 18 years of age or older and able to lift at least 50 lbs.
  • Be able to clean, provide personal care and a safe environment
  • Assist with personal care for residents (laundry, housekeeping, meal service and other related tasks) using the residents care plans as a guide.
  • Responsible for timely ordering and documenting the administration of medications and medical supplies for residents.
  • Coordinate medical & non-medical appointments and transportation for residents.
  • Perform basic resident assessment using the glucometer, blood pressure equipment, and skills appropriate for the level of care provided.
  • Ability to enter data accurately, such as physicians orders, labs, etc.
  • Ability to think critically, be detail-oriented, work well alone, handle multiple interruptions, and work on multiple tasks throughout the day.

Primary Job Purpose: To provide a pleasant, clean, and safe environment for Arbor residents. Providing the highest level of care as needed per individual living in our Assisted Living.

Must have a current QMAP certification or willing to obtain a Qmap within a reasonable time period, upon being hired.

Customer Service: Adherence to the MOSES program to include Mission, Ownership, Success, Excellence, and Service.

Essential Job Functions:

  • Practices the Values, the Mission, and Philosophy of Eben Ezer Lutheran Care Center.
  • Responsible for timely ordering and documenting the administration of medications and medical supplies for residents.
  • Assist with personal care for residents (laundry, housekeeping, meal service and other related tasks) using the residents care plans as a guide.
  • Perform accurate and complete documentation of resident information as required by federal and state regulations.
  • Coordinate medical & non-medical appointments and transportation for residents.
  • Communication of residents’ condition and significant issues with physicians, families, case manager, and Administrator as necessary.
  • Set up, prepare, and serve meals for residents. Vacuum and empty trash after meal.
  • Perform basic resident assessment using the glucometer, blood pressure equipment, and skills appropriate for the level of care provided.
  • Answers call bells timely.
  • Ability to coordinate activities with residents.
  • Exchanges information between shifts with co-workers.
  • Routinely cleans equipment and keeps work area neat and clean.
  • Identifies customers’ requirements, seeks feedback and meets customers’ expectations in a timely manner.
  • Maintains punctuality when reporting to and from work.
  • Creates and maintains a positive atmosphere throughout department and with other departments.
  • Follows established Infection Control and Universal Precautions when performing daily tasks.
  • Must maintain confidentiality of all work related information according to HIPAA regulations.
  • Assure residents rights, privacy, and HIPAA regulations are followed.
  • In addition to the responsibilities described above, the role may include other responsibilities and duties as assigned on occasion, based upon Eben Ezer needs or requirements.

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

Qualifications, Education, and Experience: High school graduate or G.E.D. CPR and First Aid certification. QMAP medication certification. Computer experience.

Non-Essential Job Functions: May participate in Eben Ezer committees. Will attend facility meetings and department meetings as instructed.

Material and Equipment used: General office equipment such as phone, fax, copier, and computer. General household equipment and supplies, such as stove, oven, dishwasher, vacuum cleaner, and household cleaners. Resident care equipment such as the glucometer, blood pressure equipment, weight machine, resident lifts, cleaning equipment, oxygen tanks and others.

Working Environment/Physical Activities: Must be able to sit, stand, and walk 8 hours a day. Must be able to lift and carry 0-50 pounds by self regularly and 50+ pounds occasionally with assistance. Sits, stands, bends, walks, reaches, lifts, keyboarding, pushing carts and wheelchairs, and moves intermittently during working hours. Must be able to push/pull up to 250 pounds regularly using a wheelchair or cart. Employee is subject to physically ill residents as well as hostile and emotionally upset residents and/or families. Employee is exposed to different demands from a variety of people and must be able to handle them professionally.

Working Hours: 8 hour shifts as assigned and filling in when necessary to cover call in’s or vacations. May include any combination of day, evening, and night shifts. Must be able to work on holidays and weekends. Schedule may vary, based on needs and supervisor discretion.
