The NP will work with chronic kidney disease patients (CKD), helping them navigate their health as they progress into End-Stage-Renal-Disease (ESRD), manage comorbidities, provide frequent counseling and education to reduce hospitalization days
The NP will establish comprehensive care plans, provide documentation of visits, and work with the dialysis interdisciplinary team to promote best outcomes for dialysis patients
The NP will be required to work in two different Nephrology Practice locations (Lompoc and Santa Barbara) to see CKD and outpatient dialysis patients.
Requirements
· Active California Nurse Practitioner License
· Current Basic Life Support (BLS with AED) certification
· Current driver’s license and insurance within state of residence required; reliable personal transportation
· Preferred 2 years experience in nephrology, intensive care, primary care, emergency, or cardiology.
Bilingual (English and Spanish) Preferred
· Demonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation of chronic patients required
· Proficient with EMR, MS Word, Excel, and Outlook
· Must be willing to drive to multiple locations
