Clinical Pharmacist – Perfect Health – Pittsburgh, PA

The Clinical Pharmacist is the expert in drug therapy for the local market. They are responsible for ensuring that the market maintains quality from a drug perspective and will coordinate any initiatives or efforts in this area. This may include working with pharmacies or providers outside of the clinic or with health plan partners. They will conduct medication therapy reviews on high-risk patients and change drug therapy under protocol. They will also assist the regional Medical Director with ad hoc projects and drug information requests as necessary.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Coordination of pharmacy-related quality initiatives for the general population and at the local level.
  • Creation of clinical programs as necessary to support quality initiatives.
  • Medication regimen review and medication reconciliation (Medication Therapy Management) for high-risk patients telephonically and in the field.
  • Collaboration with providers/team to close HEDIS/STARs gaps.
  • Practice under protocol to make changes to drug therapy.
  • Organize training/education to other practitioners on pharmacy updates and best practices.
  • Occasional travel may be required.

QUALIFICATIONS

  • Must hold an active, unrestricted pharmacist license in the state of practice.
  • At least 3 years of pharmacy experience OR ASHP-accredited PGY1 Residency.
  • Experience in ambulatory care, managed care or other clinical setting.
  • Strong written/oral communication, ability to interact effectively with multidisciplinary team.
  • Motivated self-starter that takes initiative and follows through on tasks.
  • Medication Therapy Management (MTM) experience preferred.
  • Bachelor’s degree and PharmD preferred.
  • Knowledge of managed care and Medicare/Medicaid a plus.
