Integration Specialist – Wallowa Valley Center for Wellness – Enterprise, OR

POSITION TITLE: Integration Care Coordinator

REPORTS TO: Operations Officer

FLSA STATUS: 1 FTE/ Non-exempt

Salary: DOE

POSITION SUMMARY: Provide care coordination, case management services, including but not limited to: completing community health worker pathways; assisting clinicians with tracking physical health screenings; providing support to maintain the consumer’s preferred level of independence and community integration.

DISTINGUISHING CHARACTERISTICS: Be patient and comfortable relating to individuals with mental health and/or addiction issues. Be proficient in coordinating services with care providers from other agencies. Willing and able to provide adequate written documentation of services rendered in order to satisfy Federal and State requirements.

ESSENTIAL DUTIES:
Work closely with clients, client’s families and community partners to obtain services for clients including financial and housing assistance, transportation, mental health services, medical services, social services and educational services in accordance with goals set forth in individualized care plan.

Assist consumers with medication management and monitoring medical issues, tracking status on CCBHC metrics and other grant funded programs.

Provide times and accurate documentation for billable services provided to clients in accordance with WVCW, State and Federal policies

Participate in Team Meetings and help develop workflows for grant funded programs

Provide Outpatient Case Management if needed

Provide Community Health Worker services if needed

QUALIFICATIONS:
Experience with and/or understanding of the special needs of persons who have a history of mental or emotional problems.

Good writing and speaking skills.

Past experience working in the human services field.

Familiarity with mental health and medical terminology.

Ability to learn and proficiently navigate electronic health record and other computer-based data platforms.

Other desirable personal characteristics would include tolerance, tact, the ability to control emotions in stressful situations and demonstrated functional maturity.

EXPERIENCE AND TRAINING:
A bachelors degree in a behavioral sciences field, or a combination of at least 3 years relevant work, education, training or experience and has the competencies necessary to communicate effectively;

Community Health Worker certification or willingness to attain CHW preferred

QMHA or willingness to attain QMHA certification preferred

Understand mental health assessments, treatment and service terminology and to apply the concepts;

Provide psychosocial Skills Development;

Able to implement interventions prescribed on a Individual’s Services and Support Plan within their scope of practice;

and the ability to perform the duties listed under “Principal Duties and Responsibilities”.

CONFIDENTIALITY: This position requires the handling of highly confidential information. Must adhere to rules and laws pertaining to client confidentiality.

COMMUNICATION SKILLS : Must be able to communicate information in both oral and written form with the ability to present and exchange information internally across teams and co-workers, and externally with clients and the public, Information communicated ranges from routine/ basic information to complex and confidential information

LEADERSHIP & TEAMWORK: Must be able to work effectively and constructively with other office staff, clinicians, the administrative team and all other staff. Participates in staff meetings.

PHYSICAL DEMAND: This position requires you to be regularly available for work as scheduled. Position requires professional and personal skills to cope with stress associated with work involving a high degree of mental, emotional and physical demands. This position also requires the ability to bend, stoop, push and pull. While performing the essential duties of this job, the employee is regularly required to sit; talk and hear; use hands and fingers and handle or feel; reach with hands and arms. WORK ENVIRONMENT: Work is performed in an office environment and the noise level is usually moderate, but occasionally may be exposed to loud noise such as raised voice levels and alarms. Occasionally the work environment may extend to other community offices, residential houses, and off-site locations.

This position may be exposed to the everyday risks or discomforts which require normal safety precautions typical of such places as an office (i.e., moving mechanical parts, airborne particles, and electrical shock). This person will be serving clients from all demographics within the community.
