THERAPIST – MSW PD – COMMUNITY HEALTH – Hoag Memorial Hospital Presbyterian – Newport Beach, CA

The Therapist is responsible for the assessment, coordination, planning, and implementation of psychosocial treatment for patients being receiving treatment and care across the continuum. This includes providing case management services and short-term individual, marital, and family psychotherapy. Has strong experience with clinical assessments and intervention as well as solid problem-solving skills, effective verbal and written communication skills as well as the ability to work comfortably with culturally diverse patients and strong crisis intervention skills. Maintains working knowledge of community-based resources, programs and social services. Projects an image of professionalism in communication, appearance and conduct. Participates in the continuous quality improvement process.

Essential Functions:

Case Management

  • Provide ongoing coordination of patient’s needs through the continuum of care
  • Provide ongoing psychosocial support to patients and their families for adjustment to illness, grief, psychosocial stressors and optimization of well-being during treatment and survivorship.
  • Provide ongoing review of financial, family and psychosocial support in order to identify any barriers to the treatment and termination/discharge plan.
  • Adheres to HIPPA guidelines upon communication and coordination of resources in the community to meet the discharge and psychosocial needs of the patient.
  • Educates patients and families on their health care options and connect them to resources to meet their needs.
  • Assist patient to advocate for self to receive appropriate services.
  • Works collaboratively with interdisciplinary team, including Physicians, Nurses, and other clinical staff to facilitate clinical pathways and achievement of desired treatment outcomes.

Assessment

  • Utilizes high-risk criteria in addition to information obtained from medical records, school records, clients, families, nurses, physicians, teachers and others as appropriate to evaluate patient’s biopsychosocial needs.
  • Assessments demonstrate understanding of biopsychosocial and psychiatric issues and the impact on patients and their families.
  • Assessments verify recognition of patient/family knowledge deficits regarding psychosocial aspects of illness/recovery, crisis, continuing care and community support resources. Conducts assessments related to legal and ethical reporting requirements (i.e. suicidality, homicidal tendencies, child abuse, elder abuse).

Therapeutic Interventions

  • Based on biopsychosocial assessments and recommendations for ongoing care, will develop a treatment plan and a comprehensive termination of treatment/discharge plan Provides psychotherapy services in individual, joint, family, or group format.
  • Provides non-pharmaceutical pain and stress management treatment modalities.
  • Utilizes comprehensive assessments to formulate accurate and complete treatment plans that include treatment goals and interventions that reflect psychosocial needs and diagnosis.
  • Provides therapy services that are culturally sensitive and appropriate.
  • Utilizes crisis intervention techniques to calm, comfort and stabilize highly stressed patients.
  • Works effectively with patients who may represent pediatric through geriatric developmental stages.
  • Conducts psychotherapeutic support groups and psychoeducational workshops as needed. This includes curriculum development.
  • Abides by all mandated reporting laws and protocols.
  • Serves as a valuable preceptor to MSW and MFT interns.

Documentation and Time Management

  • Documents objective, accurate and relevant data regarding patient assessments and interventions; documentation demonstrates knowledge and understanding of biopsychosocial factors germane to the patient.
  • Documentation reflects patient participation in assessment, therapy, education, and referral activities.
  • Documents pertinent client issues, contacts and termination of treatment/discharge plans
  • Plans, prioritizes and manages work assignments effectively.
  • During light work load situations, seeks out additional tasks/duties for the department.
  • Performs other duties as assigned.

Collaboration

  • Develops and sustains positive working relationships with staff, partner agencies, and referral sources in order to promote open communication channels, obtain/disseminate information, augment treatment plan, and enhance customer relations.
  • Maintains working knowledge of community-based resources, programs and social services.
  • Participates in team conferences and collaborative meetings and serves as a Hoag Hospital representative into the community
  • Shares knowledge and information appropriately with other professionals or as a member of a multidisciplinary team (as allowed for by the legal/ethical limits of confidentiality). Generates referrals and resources through networking.
  • Develops workshops, groups and other programs that leverage our community partners.

Education, Training and Experience

Required: Therapist- Social Worker or Marriage Family Therapist Required: Master’s Degree MSW or MFT from an accredited school.

Skills or Other Qualifications

Required: Strong clinical assessments, crisis intervention, problem solving, case management and mental health clinical skills. Basic computer skills, effective verbal and written communication skills. Ability to work comfortably and effectively with culturally diverse clients. Working knowledge of community-based resources, programs and social service. Decisive and capable of exercising good judgment under pressure.

Preferred: Bilingual or multilingual candidates.
