BACI Home Health is ACHC Accredited, Medicare Certified and licensed by PA Department of Health. Our office services adult and geriatric patients in Philadelphia, Bucks, Montgomery and Delaware counties.

Registered Nurse (RN) Duties:
- Follow patient’s plan of care in accordance to doctor’s orders
- Travel to patient homes as assigned and manage own schedule
- Home care visit duties may include: medication administration, wound care, catheter changes, disease management and education
- Monitor, observe, evaluate and manage patients’ conditions, report to physician and properly document
- Perform patient assessments, evaluations and Start of Care Admissions
- Case manage, follow up and coordinate with care team, family and physician
Qualifications for a Registered Nurse (RN):
- A current license as a Registered Nurse in good standing and able to practice in Pennsylvania.
- A minimum of one year of recent, verifiable RN clinical (medical/surgical) experience.
- A Minimum one year prior home health care experience providing home visits
- Graduation from an accredited and approved nursing program
- Strong interpersonal skills, excellent work ethic and communication skills
- Experience with electronic medical records (EMR) preferred. Knowledge of Axxess software is a plus.
- Knowledge of OASIS documentation
- Knowledge of Medicare requirements and compliance
- Current CPR certification
- Clear criminal background history and clear drug screen
