Patient Engagement Advisors (PEA) is seeking a full time Pharmacy Technician / Transition Specialist for our Transitions of Care “Care 360” program at AdventHealth Ocala.
Key attributes for success in this role include strong communication skills and the capacity to work with patients, physicians, pharmacists, case management, and retail personnel with genuine care and empathy.
This position operates in a hospital setting, requiring the ability to speak/hear and ability to be on feet frequently (walking and/or climbing stairs 8-10 hours daily throughout a health system facility).
The candidate hired for this position will work:
Tuesday – Saturday or Sunday – Thursday 10am – 6pm
The Transition Specialist is responsible for logistically organizing and delivering discharge prescription medications, self-care products, and health management resources to patients and/or their caregivers.
The goal of the Transition Specialist is to ensure patients successfully transition from the hospital to their next stage of care. These efforts will occur prior to the patient being discharged from the hospital.

Requirements:
- High School Education or equivalent required
- 2 years pharmacy technician experience required
- Pharmacy Technician Registration in the state of Florida (RPhT) required
- Associate’s degree (Healthcare, Business or a related field) preferred
- Retail pharmacy experience preferred
