RN Case Manager – Home Health

Schedule:
- Day shift, evening shift or night shift, flexible hours (3 12’s or 4 10’s)
- PRN weekend options available
Compensation: $36-$42/HR, dependent on experience

RN Case Manager Responsibilities:
- Interact with an interdisciplinary team to help coordinate in-home services ordered by Physician partners
- Help patients recover from acute illnesses
- Start and manage PICC’s, peripheral IV’s and midlines
- Educate patients, caregivers and family members on in-home best practices
- Help develop individualized care plans and ensure appropriate follow up through the care continuum
RN Case Manager Qualifications:
- Associate’s Degree in Nursing (RN), Bachelor’s Degree in Nursing (BSN) preferred
- An active Registered Nursing license in the state of Arizona (RN)
- 3+ years of acute care experience, preferably in the ED, ICU or Med/Surg
- 3+ years of case management / care coordination experience, preferred
- Home health experience, preferred
Job Type: Full-time
Pay: $36.00 – $42.00 per hour
