Hospice RN – Home Health PRN – ADMT Solutions – San Antonio, TX

Seeking a caring, experienced Hospice Nurse:

Hospice is an opportunity to use your skills to make a positive impact on families and patients. If you are an experienced RN, join our team to serve patients and their families. The Hospice RN will plan, implement, and evaluate the nursing care of Hospice patients, working with families, caregivers, and other members of the interdisciplinary team.

The main responsibilities are to administer skilled nursing care for clients in their place of residence. Duties include coordinating care with the interdisciplinary team, patient/family, and referring agency.


REQUIRED Qualifications:

  • Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
  • Licensure: Current state license as a Registered Nurse, current State Drivers License.
  • Experience: Two years’ experience as a Registered Nurse in a clinical care setting preferred, hospice or home health preferred.
  • Skills: Knowledge of generally accepted standards of practice. Excellent interpersonal skills and ability to communicate effectively. Proof of Hepatitis consent/declination. Specialized skills preferred in palliative and end-of-life.
  • Transportation: Reliable transportation. Valid and current auto liability insurance.


PREFERRED SKILLS:

  • Administrative: Q&A, Goal-setting, Scheduling.
  • Home health experience (non-hospice).
  • Supervisory experience.


Duties/Responsibilities:

  • Evaluation of Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patient’s physical, functional, psychosocial, and environmental needs and ADLs, i.e. risk for grief, cultural/spiritual needs.
  • Implement the individualized POC and recommend revisions to the plan as necessary.
  • Consulting with and educating the patient/family regarding disease process, self-care techniques, end-of-life care. Providing training to other staff as needed.
  • Initiating appropriate preventive and rehabilitative nursing procedures.
  • Preparing clinical and progress notes that demonstrate progress toward established goals.
  • Coordinating all patient/family services and prioritization of needs with the members of IDG.
  • Use of case management approach and referring to other services as needed.
  • Informing physicians and other personnel of changes in the patient’s needs and outcomes of the intervention, while evaluating patient/family response to care.
  • Determining the scope and frequency of services needed based on acuity and patient/family needs.
  • Assessing the ability of the caregiver to meet the patient’s immediate needs upon admission and throughout care.
  • Evaluating own needs for support and using identified systems to meet the need.
  • Communicating information using the correct process and technology available to the organization.
  • Applying specific criteria for admission ad re-certification to hospice care to establish appropriate levels of care and the patient’s eligibility.
  • Perform on-call responsibilities and provide on-call service to patients/families as assigned.


Job Types:
Full-time and Part-time.


Schedule:
Monday to Friday and weekends as needed.


Experience:

  • EMR knowledge such as AXXESS/well sky/Kinnser: 1 year (Preferred).
  • Hospice: 1-year minimum (Required).
  • RN: 1 year (Required).


Language:
Spanish (Preferred).
