Requirements of an RN Case Manager:
- 3-year RN experience preferred
- Minimum 2 Years Home Health/Hospice RN Case Manager experience REQUIRED
- Experience in EMR; McKesson, Oasis REQUIRED
- Can work M-F
- Valid driver’s license, own vehicle and vehicle insurance
- Must have Active VT State RN Licensure
- American Heart Association issued BLS/ACLS
- Must have within 12 months Physical and PPD
