Program Coordinator – Adult Day Habilitation – Amego, Inc. – Attleboro, MA

PRIMARY DUTIES

Manage the Day Habilitation Program in such a manner as to ensure that support services and community inclusion opportunities are maximized for individuals receiving services. This includes ensuring group activities are routinely planned and carried out and that staff are trained in delivering appropriate developmental activities to individuals receiving services.

Provide positive, innovative and effective leadership and motivation within your department to ensure that our organization is a responsive and professional organization, that it carries out our mission well and that our stakeholders perceive it as such. This includes periodic physical spot checks of all outside work sites and/or volunteer sites. Verbal spot checks with employers and/or volunteer organizers should be done periodically as well. Documentation of all spot checks should be maintained. This is to ensure the proper delivery of services for both the work site and the volunteer site. The minimum is two checks a month. This includes regular supervisions, staff development and mentoring, as well as managing employee relation issues that arise. Ensure that disciplinary issues are addressed within the parameters of the Progressive Discipline Policy and in coordination with Human Resources/Labor Relations.

With relevant program staff, evaluate clinical and programmatic needs. Ensure implementation of appropriate curriculum so as to address individual needs within the context of available resources.

Design and maintain an effective organizational structure, which facilitates the efficient implementation of programs. Delineate lines of authority, responsibility and communication. Establish well defined, documented, & properly communicated staff roles and ensure that appropriate staffing levels & qualifications are met. All staff must know the mission of the agency while supporting individuals in the community.

Ensure that the Day Habilitation Department operates within established budget guidelines.

Participate in individual intake, discharge & support planning. Build solid relationships with provider communities, funding sources, families and residences. Provide tours of facility and programs as necessary.

Review service/program organizational design as needed to ensure that appropriate structures are in place to meet the needs of the agency.

Ensure the implementation of licensing and accreditation standards and compliance with CARF, MASS Health, DPH and Medical regulations as applicable.

Ensure that all individuals receive professional, individualized services based upon periodic assessment of needs, and that all programs have measurable written goals & objectives with programs written and implemented in the day program. All necessary information/documentation must be completed and made available as directed by HCSIS completion guidelines. There can be no deviation from this area-all required reports must be submitted on time, there are no exceptions.

Ensure appropriate representation of pre- DHSP and DHSP meetings, as well as attending meetings relevant to Day Habilitation needs, such as quarterly meetings, funding sources, etc.

Ensure that member records are maintained according to regulations set forth by the Commonwealth of Massachusetts/Mass Health: 130 CMR 419.000 as well as CARF standards also. All documentation must be current and complete.

Maintain caseload of individuals as assigned by the Program Director.

Facilitate coordination and supervision of specialists (e.g. speech, O.T., P.T., psychology, therapist, etc.).

Demonstrate a commitment to comprehensive staff training by implementing a variety of ongoing trainings for all levels of staff. Ensure that morning meetings are conducted regularly for all staff.

Provide effective direct supervision & support to all assigned staff and ensure that your supervisory staff is providing supervision and support to their subordinates. Ensure that all staff are valued and treated fairly, with dignity and respect. Must complete supervision and performance appraisals as required with no exceptions.

Meet or exceed regulatory requirements as it pertains to day habilitation programming, which include: Member Records, CARF standards, Department of Public Health & MASS Health. Work collaboratively with Program Director in developing, reviewing and revising operating policies and procedures on an ongoing basis.

Complete quarterly audits of day habilitation programs to ensure full compliance with all regulatory agencies.

Participate in preparation of well-written proposals and grants. Maintain knowledge of state contract procurement regulations.

Maintain positive and effective relations with families, guardians, state agencies and other entities that purchase our services. Along with the Program Director, develop and implement a communication system to ensure that all guardians/parents and other stakeholders are kept updated on new activities, employment opportunities, focus of program and participation of the individuals. This should be done on a consistent basis, at least monthly.

Assist Program Director in developing an annual budget for the Day Habilitation program. Monitor & control revenues/expenses.

Ensure that Management Plans are prepared which provide analysis of program evaluation data & recommendations for corrective action as well as comprehensive annual plans which address programmatic, administrative, marketing and human resource development activities.

Assist in managing the marketing of our services by keeping abreast of trends in the field and needs of prospective individuals. Maintain ongoing efforts to publicize our services and network effectively with funding sources and school system personnel for those individuals turning 22.

Participate on committees and boards that pertain to Day Habilitation programming and regulations.

Departmental Specific Duties

Adhere to goals and objectives in securing meaningful community based volunteer and recreational options. This includes creative options for community recreational activities and skill building curriculum completed in the day program environment.

Ensure that day habilitation program is kept in a safe, clean and attractive manner and that all safety programs and policies are communicated to staff and are implemented and practiced consistently. This includes assisting in the maintenance of program vehicles..

Oversee and/or participate in the weekly on-call services on a rotational basis when possible.

Perform any additional duties as assigned by the Program Director of Adult Day Services.

QUALIFICATIONS:

Bachelor’s Degree preferred. Must have 3+ years’ leadership and management experience in a human service

  • Demonstrated experience in change management and innovation, strategic planning, financial management, organizational growth and expansion.
  • Effective communicator, with experience developing decision-making processes as a facilitator.
  • Demonstrated commitment to valuing, nurturing and celebrating diversity within the organization for staff and individuals and belief that cultural competence drives quality.
  • Mentor who nurtures and encourages the growth and development of all staff.
  • Excellent attention to detail; Organized and efficient.
  • Creative with ability to visualize growth and development with employment services.
  • Comfort and capacity to work within a highly decentralized delivery system and centralized administrative service structure.
  • Understanding of financial planning, budgeting processes with multiple funding streams, balance sheets and financial statements.
  • Familiarity and experience with Massachusetts adult day habilitation (DDS, MASS Health, DPH CARF.).
  • Ability to work flexible hours separate from the Monday through Friday schedule when needed.
  • Maintain the following certifications/licenses: CPR/First Aid; SOLVE; MAP; Drivers License
