Summary:
The Process Improvement Coach utilizes an evidence-based approach to facilitate Process Improvement methodology and tools within practice, systems and other settings. You will be accountable for facilitating the implementation of practice baseline assessments, reporting measures and ongoing facilitation and training of process improvement methodologies; achieving measurable outcomes and regular reporting to leadership.

The Process Improvement Coach is responsible for working with systems, provider groups and practices to facilitate practice redesign efforts, providing facilitation, in-person trainings and remote support towards improved process and clinical outcomes, decreased costs, and increase in efficiency that results in improved patient experience and provider and health care team satisfaction.

This candidate must live in the Rio Grande Valley, TX market area. This is a Work at Home position near Rio Grande, Valley, and Texas with 30% travel locally.

Responsibilities:
The Process Improvement Coach serves as the primary liaison, teacher, and facilitator for practices participating in practice transformation activities.

Establish and maintain a supportive, collegial role with assigned practices and their individual process improvement team members.

Facilitate practices using a standardized approach based on program model.

Develop innovative tools and resources, such as toolkits, guides, and presentations to meet current or future practice needs.

Demonstrate the ability to effectively lead and facilitate improvement teams toward measurable outcomes with increasing practice independence over time.

Train, develop and educate leaders, providers and staff in practices and identify interventions with support from your Manager to facilitate success in practices.

Routinely assess, document, and report practice progress throughout the transformation process and proactively bring practice issues to your manager.

Participate in the planning, hosting, development, presentation and delivery of formal presentations or trainings and local or national conferences/events as requested.

Continually review and manage your own time on projects and organizational work, focusing on high value activities, excellent performance or internal improvements/efficiencies that allow for work/life balance.

Qualifications:
Travel is an essential element of this role, with day travel common, and expected to be approximately 30% throughout the Rio Grande Valley.

B.A./B.S. in health care or related field and five years’ experience in primary care, ambulatory care or other related field working with providers Commensurate experience

Preference will be given for a Master’s degree, and/or a strong background in one or more of the following: quality or process improvement, data analysis, Patient Centered Medical Home (PCMH) National Committee for Quality Assurance (NCQA)recognition, change management and Medicare Advantage.

Excellent presentation & training delivery skills

High degree of professionalism: being on-time, proactive and able to manage unexpected situations

Excellent interpersonal & relationship- building skills with team members & clients

Work autonomously in a virtual environment with on-time completion of deliverables

Computer Skills:
Excellent computer skills required, particularly related to Microsoft applications Word, Visio, PowerPoint and Outlook.

Excel- must be intermediate to advanced

Excellent verbal communication and report writing skills

Ability to manage time and stress to meet competing demands

Critical thinking and analytical skills
