The Care Coordinator Assistant is responsible for assisting in multiple activities related to patient care and Chronic Care Management and Transitional Care. This includes transitional care coordination, referrals for specialists, DME and diagnostics and assist with the development, implementation and monitoring of individual patient centered care plans. The CCA interacts with other team members concerning patient needs identified during their day to day activities.
We are a unique office practice, and therefore we are looking for candidates that possess a unique skill set, including the ability to be a self-starter, display excellent phone skills and etiquette, provide excellent customer service, multitasking, and excellent organizational skills. Experience with senior patients preferred.
