Housing Specialist- FACT II- 00159 – Community Bridges Inc. – Phoenix, AZ

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

We currently have an opening for Housing Specialist for our FACT-II team within Maricopa County, based out of Phoenix.

The Housing Specialist is the member of the team responsible to assist SMI clients with criminal records in obtaining permanent supportive housing. The Housing Specialist is expected to work with community supportive housing agencies on temporary and permanent housing opportunities, and will assess clients for readiness and recommend in home supports for wrap around services and skills training. He/She will also provide in field support to clients and work with landlords to ensure ongoing safety to the client as well as the appropriateness of the placement.


Minimum Requirements:

  • Bachelor’s degree in a field related to behavioral health OR a combination of education, training, and/or experience meeting CBI Clinical Policies and Procedure standards for BHT.
  • Minimum of 1 year experience working with the SMI population with additional experience in multiple settings and levels of care, including both mental health and substance abuse.
  • One year experience in a behavioral health position providing outreach and engagement activities.
  • Ability to pass criminal background check for jail visitation/clearance.
  • Valid AZ Driver’s License with 39 month clear MVR
