Social Worker, Counselor, Case Manager for CRU/Detox – Seneca Health Services, Inc – Maxwelton, WV

QUALIFICATION REQUIREMENTS:

Bachelor’s Degree in a Human Service field required. West Virginia Social Work License preferred.

One year experience working with individuals diagnosed with severe and persistent mental illness and/or substance abuse is preferred.

Valid driver’s license and vehicle required.

Must pass a Criminal Background Check and Employment Fitness Determination through WVCARES and a drug screen.

Demonstrated knowledge of basic computer skills required.


PURPOSE:

Provides a full array of crisis stabilization services including, but not limited to, individual/group supportive counseling; development/implementation of Individualized Treatment Plans; and completion of admissions and
discharges. Responsible for ensuring other program and physical needs of the consumer are met and a therapeutic
environment is maintained to stabilize the condition of a consumer immediately following a crisis episode.


ESSENTIAL FUNCTIONS:

  • Provides individual, group and/or family supportive counseling.
  • Completes initial intake process, discharges, treatment plans and other required paperwork.
  • Assists in the evaluation of referrals for potential admission into the facility.
  • Completes referrals for admission accurately and thoroughly, ensuring appropriate symptomology and information is gathered during the referral process.
  • Coordinates and participates in Interdisciplinary Team Meetings, daily staffing, physician rounds, and discharge planning.
  • Coordinates family involvement in treatment, if applicable, and provides family supportive counseling during weekends, as needed.
  • Assures all documentation is accurate and submitted according to Seneca policy.
  • Interacts positively with consumers and models appropriate socialization and communication skills.
  • Reports and documents any unusual events or violations of policy to the appropriate person.
  • Attends all staff meetings, Seneca required training, workshops or seminars. Maintains current certifications, including but not limited to CPR, First Aid, CPI, Food Handling and Infection Control.
  • Represent Seneca in a positive manner including maintaining a positive attitude, performing duties in a manner in accordance with Seneca’s Mission Statement and Code of Ethics and treats consumers with dignity and respect.
  • Utilizes Personal Protective Equipment and follows guidelines outlined in Seneca’s Exposure Control Plan.
  • Other duties as assigned by supervisor.
