Experienced in Long Term Care. Knowledge of PDPM. Understand the documentation. Responsible for Care Plans, Assessments, Evaluations, and Certification of Stay. Will work closely with the IDT and families to achieve the best possible planning and outcome for residents. Work closely with CNAs to ensure ADLS is completed and charted properly. Demonstrate leadership and management ability. Must have Texas license in good standing. Other job duties as assigned. Salary requirements discussed upon the interview.
