POSITION SUMMARY:

The Community Health Outreach Worker is responsible for conducting outreach activities at shelters and on the streets, for assisting providers with medical services at shelters, on the streets and setting up temporary exam areas, assisting the clinicians and preparing the patients for examination. The CHOW is also responsible for representing Care Link and CMC to other homeless service providers, individuals, community groups and partners. The Community Health Outreach Worker reports to the Care Link Coordinator.

MINIMUM REQUIREMENTS:

· One year experience in clinical setting assisting providers in delivery of medical services and/or satisfactory completion of a medical assistant training program

· High School Diploma or GED

· Current certification in basic cardiopulmonary resusci­tation

· One (1) year experience in community outreach is desirable

· Valid California driver’s license, proof of auto insurance, and personal transportation

SPECIFIC DUTIES:

· Prepare examination areas, immunizations, instruments, and set up for procedures both in clinic and in outreach settings

· Prepare patients for examinations including vital signs, perform audio/visual testing, and obtain consents for procedures

· Translate for providers and other staff

· Give oral, intramuscular, intradermal and subcutaneous injections and immunizations as ordered by clinicians

· Perform and record urine dips and prepare urine samples for laboratory

· Instruct patients in use of inhalers, glucometers and related items as directed by clinician; may deliver medications to individuals unable to come to clinic

· Assist in maintaining supplies in cabinets, drawers, crash carts , outreach bags and storage room, checking for expiration dates as applicable

· Clean and sterilize supplies as directed. Prepare and stock examination rooms for use

· Consults with and advise the Care Link Coordinator regularly on community activities and completes daily activity reports

· Conducts street outreach activities (that may include extended working hours) with outreach staff of other agencies and indigenous leaders to identify individuals with medical needs and to provide information regarding Care Link and other community resources available

· Serves as departmental representative at various meetings when assigned

· Serves as program liaison with community partners and other service providers

· Schedules and conducts presentations to groups regarding homelessness and health, CMC history and organization. Presents information to community agencies and partners about programs, services and needs

· Attends and participates in staff meetings and training as required by supervising staff

· Assists with various education and informational activities that are assigned and works as a team member to ensure success of community and other events

· Responsible for gathering and assembling related information and preparing data collection reports, and translating written material related to the project when necessary

· Performs other tasks as assigned

PERFORMANCE REQUIREMENTS

Knowledge, Skills and Abilities:

· Knowledge of routine medical clinic practices

· Knowledge of geographical area and ethnic communities

· Knowledge and understanding of customs, beliefs and needs of consumer group(s) served

· Knowledge of community resources and referral agencies

· Knowledge of computer systems and applications

· Knowledge of grammar, spelling and punctuation to compile and type reports

· Ability to communicate effectively in English and Spanish written and verbal, commensurate with high school levels is highly desirable

· Skill in interviewing and communicating to elicit information and cooperation from patients

· Skill in establishing and maintaining effective working relationships with patients, staff, and the public

· Skill in public speaking

· Ability to prepare routine records and reports using terminology common to outreach programs

· Ability to interpret, adapt, and apply appropriate written guidelines and work practices

TYPICAL PHYSICAL DEMANDS:

May require sitting for long periods of time. Occasional bending, stretching or lifting. Requires frequent walking occasionally on uneven terrain and standing for prolonged periods of time. Lifts and push/pulls items weighing in excess of 50 pounds. Vision must be correctable to 20/40 and hearing must be in the normal range for telephone contacts. May require contact with individuals with communicable diseases. Requires the ability to work under stressful conditions. Requires ability to drive to outreach areas for presentations and other activities. May require working week nights and weekends.

TYPICAL WORKING CONDITIONS:

Normal office environment and frequent work performed outside or in shelters with exposure to rain, heat, dust, wind, pesticides, pollens, and other weather demands common to a labor camp, park, community center or health fair. Exposure to individuals who are violent, abusive, mentally ill and/or under the influence of alcohol or drugs.

Job Type: Full-time

Pay: $14.50 – $16.50 per hour
