Mercy Home Health Care has a need for a Full-time RN Case Manager for the Suburban branch (Montgomery County).
Manages, oversees and provides primary patient care delivery to a select group of patients supporting the patient centered care model, assuring quality and maintaining open communication. Presents the patient at interdisciplinary team meetings as scheduled and as needed. Collaborates with team members to assure patient care delivery needs are met. Interacts with any and all members of the care group both internal and external to the organization as needed.
BSN; Valid PA Nursing License
1 year of Med-Surg experience required. Recent home care experience preferred. Currently licensed to pratice as a Registered Nurse in applicable state.
Working knowledge in navigating through Excel and utilizaiton of Outlook and Word. Excellent communication and problem solving skills. Excellent organization and time management skills. Ability to multi-task. Excellent interpersonal skills and ability to work within an Inter-Disciplinary Team approach. Self confident and assertive, flexible, reliable and dependable. Positive, can do attitude. Currently licensed or eligible for a license to operate an automobile in the applicable state. CPR certification. Regular availability to work – will be required to work a rotating weekend and holiday schedule.
