SUMMARY

Under general direction and supervision of the site supervisor/manager or an operational administrative staff employee, performs a variety of work consisting of routine difficulty in providing services/supports to consumers with developmental disabilities. The nature of the services range from various personal care activities to activities outlined in an Individual Support Plan (ISP). Performs miscellaneous related work as required.


MINIMUM QUALIFICATIONS:

  • Age: Must be at least 18 years of age (licensing requirement). If driving is required for the position, must be 21 years old with no more than two (2) minor moving violations or one (1) accident within the past three (3) years.
  • Experience: Any combination of training and experience needed to provide direct care services; some funding contracts require previous DSP experience (the amount of experience is based on each specific contract).
  • If position requires driving a CPES vehicle or a rental car, must possess a current/valid Arizona driver’s license and be insurable under the agency’s automobile policy.
  • Have access to reliable, safe transportation.
  • Able to work flexible schedules: weekends, nights, days, evenings, partial shifts, 8/12/16 hour shifts.
  • Able to operate common office equipment and have basic computer skills, including word processing.


ESSENTIAL FUNCTIONS:

  • Provides the appropriate environment to assure the health and safety of the consumers being supported;
  • Assists consumers in performing daily living activities, including but not limited to: eating, showering, bathing, toileting, dressing, shaving, combing hair, shampooing, brushing teeth, cutting nails;
  • Assists consumers with household chores (involving many physical activities, including but not limited to kneeling, reaching, stretching, bending, etc., and the use of household cleaners).
  • Transports and/or escorts consumers to medical, vocational, residential, social activities, and runs consumer related errands;
  • Assists consumers in the self-administration of medications including non-prescription and prescription medication as prescribed by licensed medical specialists;
  • Safely and effectively monitors consumer activities, with or without a reasonable accommodation;
  • Observes and assesses consumer’s behaviors to provide positive behavioral supports;
  • Implements proactive strategies to enhance and reinforce consumer positive behaviors;
  • Assess and provides behavior/physical management techniques in crisis situations and attains assistance if needed;
  • Records and maintains records, progress notes, records daily activities in communication logs, maintains records as needed and may be required to write reports;
  • Reads and implements a consumer’s Individual Support Plan (ISP), medical/medication directions, and formalized records concerning consumer behaviors and activities;
  • Per the State of Arizona regulations, DSP’s must be able to:
  • lift a minimum of 50 pounds;
  • perform standard First Aid
  • perform Cardio Pulmonary Resuscitation (CPR)
  • perform Crisis Intervention Techniques (CIT) (may require significant physical activity)
  • fulfill Article 9 requirements (consumer rights).
  • If the consumer is incapacitated, operates equipment to aid in the assistance/movement of the consumer.
  • If the consumer uses a wheelchair, safely transfers the consumer in/out of the wheelchair as well as safely transports the consumer.
  • If the consumer is in a home that has stairs, the employee must be able to safely maneuver the stairs alone and while assisting the consumer.


ADDITIONAL DUTIES AND RESPONSIBILITIES:

  • Provides direct services in crisis or emergency situations;
  • Provides a safe environment by communicating policies/procedures with consumers, advocates and visitors;
  • Communicates and interacts with consumers on an individual and group basis promoting self-determination;
  • Encourages consumers to participate in counseling, social, recreational, vocational and other activities identified in their support plans while promoting individual choice and decision making opportunities;
  • Reviews and implements approved policies, standards, services and procedures;
  • Takes part in developing and implementing Individualized Support Plans or person-centered plans for assigned consumers and ensures directives are followed pursuant to State standards;
  • Participates in staff and administrative meetings as well as in-service trainings;
  • Reports unusual incidents on proper forms and informs appropriate staff regarding incidents involving vocational, day, social, recreational, behavioral, personal living and community services problems within required time-frames;
  • Observes and reports changes in consumer’s mental/physical health to enable intervention/prevention of problems;
  • Performs housekeeping tasks such as cleaning, cooking, laundering, making beds, yard work, and other household chores or tasks required to support/serve the consumer including physical management procedures;
  • Acts as advocate in resolving problems and securing needed services for consumers as well as ensuring personal choice, freedoms, responsibility and support for consumers;
  • Develops and maintains working relationships with all team members, including various operational administrative staff, family members, guardians, fiduciaries, advocates, etc.
  • Able to positively interact and develop rapport with consumers and their families, professional support staff, and various levels of staff from community agencies;
  • Able to maintain a calm, non-defensive, supportive attitude during crisis or potential crisis situations;
  • Must be able to endure extreme outdoor temperatures and sudden temperature changes;
  • Must be able to operate an agency transport vehicle (unless declared a non-driver or a vehicle is not required at the site);
