We are immediately hiring TWO Social Workers for our Short-Term Care/Rehabilitation Units!
Discharge Planner/Social Worker will be responsible for providing a range of social work services including:
Case Management for Discharge Planning. Following skills support being successful in the position: Experience in utilization review and communication with Insurances, comprehensive psychosocial assessments, developing a Discharge plan, using collaboration with the team, crisis and therapeutic interventions, resource referral and contact with families of residents.
Minimum Requirements:
- Discharge Planning and Case Management Experience
- Experience with patient discharge process in a Medical setting
- Understanding of Medicare, HMO’s and Medicaid
- Strong verbal, written communication and Customer Service Skills
- Time Management and Organizational Skills
- Computer skills for charting and documentation
- Strong team player with the ability to have FUN!
