Position Summary

At Stony Brook Medicine, our Social Workers are valuable members of our team, who provide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings. Social Workers plan, implement, coordinate and document treatment for individuals diagnosed with: mental illness and serious emotional disturbance; mental retardation and developmental disabilities; work-related illnesses or injuries; chemical addiction; or perpetrators or victims of neglect, abuse, or crime who need therapeutic intervention, habilitation, training, supervision and re-socialization, or confinement.

Qualified candidates will demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively respond to changing patient needs by making decisions based on ethical principles and adhering to our high standard of excellence.

The Per-diem worker will be expected to cover Pediatric and Adult Emergency Department overnight as an alternate to current Overnight Social Work. Ability to respond to multiple patient areas dependent on need; including but not limited to: ED General Medicine, Surgery, Obstetrics, Pediatrics, Oncology, Intensive Care, and Trauma.

The Per-Diem worker needs to be able to work independently, initiate, and follow through on cases.

Demonstrates excellent written and oral communication skills. Demonstrates strong Psychosocial Assessment skill set. Demonstrates understanding of discharge planning and long length of stay barriers and systems. Working knowledge of Long Island and Metropolitan NY Community resources. Demonstrates patient first, patient advocacy and service excellence. Computer literate and understanding of data analysis.

Demonstrates the following: understanding of community systems; demonstrates ability for project management and action planning; demonstrates teamwork and team leader focus; understanding of data collection and statistics. Ability to cover multiple patient population.


The Per-diem worker will support staffing for Emergency room overnight hours.


Duties of
the Per-diem Social Worker may include the following but are not limited to:

  • Psychosocial assessment of selected cases.
  • Effectuate discharge planning.
  • Round and coordinate with medical team.
  • Work with community agencies in regards to patient post discharge needs.
  • Compliance with documentation of Social Work intervention.
  • Coordination of discharge transportation, insurance verification, and advocacy with insurance companies for discharge planning and insurance reviews as needed.
  • Assist patient financial services, benefits or with Medicaid applications as needed.
  • Coordinate and give review to insurance companies as needed; contact insurance companies for authorizations.
  • Assist patients and families with advanced directives.
  • Coordinate with Psychiatry and intervention with psychiatric patients on medicine units.
  • Cross coverage on units.
  • Assist patients and families with advanced directives and MOLST
  • All other duties as assigned.


Qualifications


Required
:

  • MSW from an accredited Social Work Education School. NYS LMSW.
  • One to Three years’ experience in Inpatient Medical Hospital or Sub –Acute Setting.


Preferred
:

  • Experience in Emergency Department.
  • Experience of four or more years in an Inpatient Medical Hospital or Sub-Acute Setting.

  • Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Medicine’s Employee Health Services*
  • Complete electronic reference check with a minimum of three (3) professional references.
  • Successfully complete a 5 panel drug screen*
  • Successfully complete a Background Check investigation.
  • Provide a copy of any required New York State license(s)/certificate(s).


Failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.
