LPN – Skilled Nursing – VMP Healthcare & Community Living – West Allis, WI

SUMMARY

Under the general supervision of a Registered Nurse the Licensed Practical Nurse provides direct and indirect care to residents. Performs a variety of technical procedures in caring for subacute, convalescent and chronically ill residents. Collaborates with the Registered Nurse in administering care to the complex and unstable resident. Independent projects, learning projects or quality improvement projects are encouraged. Maintains practice under the LPN scope of practice guidelines.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Documentation

  • Independently completes all required documentation per VMP policy.

  • Performs data collection duties and documents condition and changes in the physical, psychological, social and spiritual status of assigned residents.


Education

  • LPN licensure in Wisconsin.

  • Maintains a minimum of 12 CEUs per year.

  • Completes all mandatory in-house training, including skills review and competency testing.


Leadership

  • Effectively coordinates multi-disciplinary activities.

  • Takes advantage of opportunities to explain to CNAs and others the rationale for actions.

  • Assists RN in evaluating activities of nonprofessional staff.

  • Demonstrates safe practice by example.

  • Provides leadership in identifying and addressing potentially unsafe practices.

  • Actively participates in the orientation of new employees.

  • Supports and participates in unit and facility committees.

  • Acts as a resource.

  • Effectively handles routine conflicts using a direct, tactful, approach.


Nursing Process

  • Assures that admission data collection is thoroughly completed for assigned patients. In collaboration with an RN, nursing diagnoses and nursing care plans are initiated based on data collected via MDS, database, medication review or other data.

  • In collaboration with the RN, determines health status of residents at appropriate intervals, per VMP policy.

  • Demonstrates an understanding of basic pathophysiology related to disease process and incorporates this knowledge into the plan of care.


Professionalism

  • Provides detailed input for evaluation of others and provides this input by date required.

  • Identifies challenges during self-evaluation and plans interventions of correction.

  • Changes performance behavior as a result of feedback from others.

  • Plans future goals that are both measurable and appropriate to position.


Skills

  • Independently seeks opportunities to improve nursing practice.

  • Competently performs skills as indicated on the LPN checklist. If unfamiliar with a procedure or task, consults appropriate policy/procedure and seeks appropriate assistance.

  • Provides direct resident care in an efficient, effective manner, which includes physical, psychosocial and spiritual needs of residents consistent with the plan of care, nursing process and department policy and procedures.

  • Administers medications according to policies and procedures and records each medication at time administered.

  • Responds to emergency situations addressing immediate issues and taking direction from RNs as situations warrant. Recognizes need for additional input from RN.

  • Maintains CPR certification.


Other

  • Must work well with geriatric patients, including those with memory impairment. Needs to demonstrate kindness, caring and patience with the ability to take and receive direction well in both routine and emergency situations.

  • Organizes care for residents to meet physical, psychosocial and spiritual needs consistent with established plans.

  • Discusses with Registered Nurse possible causes for changes in resident condition based on knowledge and experience.

  • Involves resident in care activities. Provides demonstration and supervision for the resident in providing simple self-care activities.

  • Participates in and documents information from conferences regarding resident care.

  • Obtains, transcribes and follows up on physician orders as appropriate.

  • Provides health care teaching and rehabilitation measures in collaboration with physician and therapists, which also includes preparation for discharge.

  • Maintains acceptable communication standards.

  • Maintains confidentiality.

  • Maintains CPR certification per VMP policy.

  • Maintains acceptable work standards which include, but are not limited to, dress code, attendance, tardiness and required in-service training.

  • Adheres to all facility and departmental policies and procedures including infection control.

  • Adheres to all State and Federal regulations and guidelines.

  • Completes other duties as assigned.

SUPERVISORY RESPONSIBILITIES

Supervises CNAs providing direct patient care to his/her assigned patients.

EDUCATION and/or EXPERIENCE

Technical or collegiate school completion in preparation for LPN licensure. Minimum of six months experience in a long term care setting following licensure is preferred.


LANGUAGE SKILLS

Ability to read, write and understand English. Ability to read, analyze and interpret professional journals, technical procedures and government regulations. Ability to write reports and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers and the general public.


MATHEMATICAL SKILLS

Ability to divide and compute percentages, add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals.


REASONING ABILITY

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.


CERTIFICATES, LICENSES, REGISTRATIONS

Valid LPN license in the State of Wisconsin.
