We are looking for an LVN Case Manager.
- Conduct review for appropriate utilization of services from admission through discharge. Evaluate patient satisfaction and quality of care provided.
- Present notice of discharge
- Communicate with physicians at regular intervals throughout patients stay and develops an effective working relationship.
- Assess patient’s progress through expected course of stay.
- Serve as a patient advocate.
- Enhance a collaborative relationship to maximize the patient’s and family’s ability to make informed decisions.
Qualifications:
- Licensed California LVN
- Must have at least one year of case management experience
