Family Centered Hospice is a dynamic post-acute care service provider of intermittent home health care, palliative care, hospice care, skilled private care services and custodial private care services.
We are seeking a dedicated Full Time Hospice Nurse to provide compassionate and quality end of life care to patient and families. Our goal is to improve the quality of life, and allow patients and families to focus on relationship, emotional, and spiritual needs.
Our RNs, plan, organize and direct all phases of hospice care with a one-on-one patient/family focus. You will monitor and regularly re-evaluate patient nursing needs, initiate the appropriate plan of care and makes necessary revisions as patient status and needs change.
Minimum Qualifications:
- A valid RN/BSN license
- At least one year of hospice experience
- Be well organized
- Comfortable with electronic documentation of medical records – Kinnser/Wellsky experience a plus.
