JOB SUMMARY:
Coordinate Medicare services by maintaining accurate and appropriate documentation. Assess, observe, implement and evaluate Plan of Care. Supervise general nursing care or residents including their medical, physical, emotional and psychosocial needs reflecting services provided per physician direction and service received.

RESPONSIBILITIES:

  • Attend admissions/clinical update meetings. Participate in pre-admission skilled screening. Determine if resident meets skilling criteria and if appropriate for this facility.
  • Initiate and implement Plan of Care, care guides, safety interventions and ensure equipment needs are known and available, for Medicare residents.
  • Process Medicare certifications.
  • Maintain Medicare tracking records in order to make available for bookkeeping by the 7th of each month.
  • Complete MDS for Medicare admits by OBRA guidelines, ensuring comprehensive Plan of Care is updated to reflect condition.
  • Care Plan adverse drug reaction, pre-restraint, restraint reduction and bowel and bladder assessment with comprehensive MDS, and as resident condition warrants.
  • Computer entry, transmission, and printing of MDS, including re-entry and discharge information and have on clinical record within 14 days of completion date.
  • Monitor telephone orders and add to care guides and Care Plan.
  • Review telephone orders, monitor infections and document resolution in Plan of Care and Infection surveillance report.
  • Monitor halls to ensure compliance to resident’s Plan of Care.
  • Schedule Care Plan meetings and maintain open communication with therapy.
  • Attend monthly PI meetings and report status to committee. Attend weekly UR meetings. Attend continuing education, monthly nurses meeting and in-service training as needed to remain current on job skills and new regulations as required by facility policy or regulatory agencies.
  • Complete and update Forms 802 and 672 daily and submit weekly, each Friday, to the Administrator.
  • Performs other duties as assigned by the DON, Administrator and/or Governing Body.
  • Complete item 14 day “look behind” for appropriate and required Medicare Charting. Submit weekly report, each Wednesday, to the Administrator and Executive Director
  • Participates in the On Call Nurse Management Rotation

Minimum Qualifications:

  • Must be as a minimum, a graduate from an accredited RN training program; BSN or MSN preferred.
  • Must have, as a minimum, three years MDS nursing experience in a hospital, skilled nursing care facility, or other related medical facility; previous nursing management experience preferred.
  • Must possess a current, unencumbered, active license to practice as an RN in the State of Ohio.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • Must be able to read, write, speak, and understand the English language. Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Must possess leadership and supervisory ability and be willing to work harmoniously with and supervise other personnel.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  • Must not pose a direct threat to the health and safety of other individuals in the workplace.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
  • May be necessary to assist in the evacuation of residents during emergency situations.

Working Conditions:

  • Works in office area(s) as well as throughout the nursing service area (i.e., drug rooms, nurses stations, resident rooms, etc.)
  • Moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
  • Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
  • Communicates with the medical staff, nursing personnel, and other department supervisors.
  • Works beyond normal working hours, and in other positions temporarily, when necessary.
  • Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
  • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
  • May be subject to the handling of and exposure to hazardous chemicals.
