Educational Requirements

  • High School Diploma or GED

Preferred Requirements

  • Experience in rehabilitation or working with those experiencing serious mental illness.
  • Experience working in residential and / or crisis management environments

Role Summary

Provide support, skill training and one on one supervision to clients as outlined in treatment plan. Report any needs client has to program coordinator, primary therapist or on-call staff as needed. Maintain case records and documentation in accordance with PWC policies and procedures. Coordinate facility operations daily with other staff and supervisor.

  • Demonstrate independent work initiative and sound judgement

Core Responsibilities

1. Provides support to clients.

2. Provides one on one supervision to clients as outlined in treatment plan.

3. Provides activity as outlined in the treatment plan.

4. Provides skill training as outlined in the treatment plan. This may include coping skills, self-care, social interaction skills, illness management, crisis management, activities of daily living, community awareness, etc.

5. Provides basic risk assessment and immediate referral to program coordinator, primary therapist or on-call staff as needed for expanded risk assessment and intervention.

6. Provides access to basic community services as necessary to stabilization, i.e. physician visits, prescriptions, family contacts, groups, as outlined in crisis treatment plan.

7. Is responsible for contacting clinical staff as needed to intervene in situations needing professional support, increased intervention, alternative placement or level of care.

8. Coordinate daily facility operations.

9. May administer Drug and Alcohol screens / tests as needed and authorized by clinical staff or primary therapist.

10.Provides transitional support for clients leaving a residential environment to their own home or alternative placement.

11.Participates as a team member of the client’s care team.

12.Attends supervision and staff meetings as required.

13.Maintains case records and other documentation as outlined in agency policies.

14.Familiarizes self and complies with all policies and procedures for PWC and the individual programs they will be working in.

15.Participates in training as outlined with and recommended by supervisor.

16.Maintains the Agency standard of expected minimum direct service productivity.
