SSVF HEALTH CARE NAVIGATOR: Immediate full-time opening, for a SSVF Health Care Navigator. Serving 26 SSVF county coverage areas with base location in Hamilton County.
DUTIES / RESPONSIBILITIES:
– SSVF Health Care Navigator is a member of the SSVF Rapid Rehousing Team, which is a VA-funded grant providing Housing First services to veteran families experiencing homelessness.
– Connecting Veterans to VA health care benefits or community health care services.
– Provide case management and care coordination across episodes of care, acting as a health coach by proactively supporting the Veteran to optimize treatment interventions and outcomes
– Assists in identifying the Veteran and family’s health education needs and materials that match the literacy level of the Veteran
– Interdisciplinary collaboration, coordination, and consultation while adhering to ethical principles about confidentiality, informed consent, compliance with relevant laws, and agency policies.
– Administrative duties and system improvement
– Work closely with Veteran’s primary care provider and members of their interdisciplinary team
– Act as a liaison between SSVF team and VA or community medical clinic
– Work with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans
– Non-clinical assessment to understand the Veteran’s situation, potential barriers to care, the causes, and the impact of barriers on their ability to access and maintain health care services
– Emphasis on community services, outreach, and referrals needed for the Veteran
– Monitor progress, maintains comprehensive documentation, and provides information to treatment team members when appropriate
– Relationship with supervisors, co-workers, program participants, visitors, and the general public is consistently courteous, cooperative, and contributes to the effective operation of the SSVF program.
QUALIFICATIONS (knowledge, skills, and abilities):
– Master’s level social worker or equivalent education and experience is preferred
– Excellent judgment
– At least two years of experience in a health care or social services area of practice
– Ability to work collaboratively within a team to provide timely, appropriate, Veteran-centered care equitably and to identify and address systems challenges for enhanced care coordination as needed
– Belief in the Housing First philosophy and the Rapid Rehousing model
Ability to provide a mix of remote and face-to-face services
