The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly) is a new non-profit that delivers medical, social, and behavioral services to the frail and elderly to the North County of San Diego. This PACE center will be entrepreneurial in nature and will focus on innovations and bringing new processes to the traditional PACE environment.

Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life and independence.

Job Summary:

Under the supervision of the Executive Director, the Social Worker is responsible for direct social work case management services as a member of the interdisciplinary team (IDT) and initial, semi-annual, unscheduled, and annual assessments; attend morning updates and report changes in participants’ baseline status to appropriate staff daily. Maintain current, written case management records, including ongoing documentation of services provided, reassessment of changing needs and patient’s expressed wishes. Works closely with family members and outside agencies.

Minimum Requirements:

Master’s degree in Social Work required.
Current California Social Work license required.
Minimum of one (1) year of documented experience in working with the frail and elderly population required.
Experience in a community-based setting or geriatric program preferred.
Experience in actively contributing in multi-disciplinary group meetings to advocate for participants’ unique and complex needs.
Excellent interpersonal and customer service skills.
Strong verbal and written communication skills.
Excellent problem-solving skills.
