PRN Home Health Field Nurse (South East Houston)

Health Vision Home Health Services is looking to fill 2 part time Registered Nurse positions (PRN).
As a PRN Field Nurse you will provide patient assessment, implementation of the plan of care and, OASIS completion.

Duties and Responsibilities:
-Provides skilled nursing care to patients requiring professional nursing services in their homes.
-Observes patients for changes in condition, reports changes appropriately to the physician, DON and other –members of the home care team.
-Initiates and maintains implementation of orders for medication and treatments as prescribed by the physician.
-Assists and instructs patients, family members or other patient representatives in the implementation of the care plan necessary to promote the patient’s health, safety, and independent living.
-Participates in case conferences with other members of the health care team.
-Participates in quality improvement/ assurance activities as assigned.

Minimum Requirements:

Current state license as a Registered Nurse.
Minimum of 1 year experience as a Registered
Demonstrated knowledge and skill in current nursing practice.
Evidence of effective communication and organization skills
Holds a valid driver’s license.
