Nurse Care Manager (RN) (Lawrence)

The Clinical Care Manager (CCM) provides care coordination and clinical management for One Care enrollees with complex healthcare needs. The Clinical Care Manager collaborates with Vinfen’s ACCS teams and the One Care Plan’s clinical staff to maximize coordination and integration of care. The CCM will be at the helm of organizing and coordinating resources and services in response to the individual healthcare needs across multiple settings.

This position will be based out of our office in Lawrence and will work primarily in the Merrimac Valley.

A master’s degree in Nursing preferred; bachelor’s degree in Nursing required. Experience may substitute for education. High school diploma or GED required.
Current RN in state of practice required.
At least 5 years clinical and case management experienced preferred.
Experience working with people with psychiatric conditions and/or co-occurring disorders strongly preferred.
Strong time management and writing skills.

✓Must be a least 21 years of age and have held a valid US driver’s license for at least one year and should be able to pass a CORI, driving record, employment reference check as well as a multi-state background check.

✓Be willing to transport clients.

✓Must be able to become CPR and First Aid certified within two weeks of hire and become MAP certified within 150 days of hire.
