Nurse Practitioner for School Based Health Center – Physician Affiliate Group of NY – Bronx, NY

NYC Health + Hospitals/Gotham Health, JHS 22 is one of Gotham Health’s school based health centers. Gotham is the country’s largest Federally Qualified Health Center operating school satellites across the five boroughs of NYC and offering optimum level of health care for their patients. Gotham offers patient centered primary and preventive care services and is focused on meeting community needs.

This position is for JHS 22 – Middle school covering 5th to 8th grade.

Responsibilities:

• Obtains complete history of patient, recording elements of past history, family history, review of health habits, environmental, and occupational data
• Conducts physical examination for the diagnosis and treatment of acute or chronic medical and reproductive health problems
• Orders and schedules tests, including laboratory, radiological and special exams in consultationl Order related treatments as outlined in approved research protocols. NPs must order with a supervising physician
• Follows-up on abnormal laboratory test results
• Acts as a preceptor for health practitioners in training including medical students, residents, nurse practitioners in training, and physician assistants in training
• Provides ongoing supervision and/or evaluation of Medical Office Assistants (MA).
• Maintains the accuracy and completeness of all patient records. Complete progress notes, procedure notes, and discharge summaries and close out the electronic medical record (EMR) within 72 hours
• Provides follow-up and health maintenance care including appropriate adjustments of medication in accordance with established protocols or, for NPs, specific instructions from a supervising physician
• Develop and implement special projects (ie. Quality Assurance Studies) to enhance the role of the SBHC within the school
• Provides additional clinical support progrmmatically as needed

Qualifications:

• Must be a Licensed Nurse Practitioner by the state of New York
• Must have interpersonal skills sufficient to effectively interact with patients, family, physicians and hospital staff in fulfilling duties and responsibilities of position
• Must have analytical abilities sufficient to interpret patient examination results and properly administer required tests
