Registered Nurse Case Manager (Upper East Side)

We are seeking for Registered Nurse to join our team for clinical, administrative and Quality Assurance efforts in Home Health Care in NY, CT, and NJ. Schedule is flexible, but at least 2 days in our New York City office are required. Salary $40/hr if Part Time or $80,000/year if Full Time.

Responsibilities:

– Going to client’s home for initial assessments and creating a plan of care on an as needed basis.
– Putting together policy, procedures, and forms and making sure they are up to date with current regulations
– Conduct phone/in-home assessments to check in with clients and caregivers
– Establish QA and compliance process
– Observe caregiver in work setting and write up performance report

Qualifications:

– Valid RN license in New York State, other states (NJ, CT) a plus!
– Previous experience in home care, nursing or other medical fields – 5 to 10 years preferred
– Administrative and organizational skills
– Valid Driver’s License
– Quality Assurance experience
– 2 excellent professional references
