Job Description

This position is for Licensed Practical Nurses who wishes to provide Home Care services. Locations available for service: Philadelphia, Souderton, Pottstown and Langhorne.

Need for LPNs. Must be able to work independently and can provide one-on-one care at client’s home in the Philadelphia area. Experience of 1 year in Hospital and/or in a Home Care setting.
Delivery of patient care as appropriate and in accordance with Plan of Care and Agency Policies.

DUTIES/RESPONSIBILITIES

Provides nursing care according to the plan of care derived from physician’s orders and assessment needs. Nursing Care includes physical care, emotional support, and patient and family education. Supervisory of Medication/Management, assist with ADL/IADL and other duties as required.
Recognizes and reports symptoms that indicate changes in the condition of the patient to the Case Manager.
Provides nursing care and services that is in accordance with Agency patient care and general medical policies.
Provides patients with the knowledge and assistance to learn appropriate self care techniques.
Accurately records patient’s condition in the Nurses notes.
Observes, records and reports to the appropriate professional the symptoms, reaction and changes including: General physical and mental condition of patients, and signs and symptoms which may be indicative of untoward changes
Must be sensitive to stresses in human relationships between patients, between patients and personnel, and between patients and their families and visitors.
Assists with the rehabilitation of patients according to the medical care plan through being aware of and encouraging the interests and special aptitudes of patients; encouraging patients to help themselves within their own capabilities in performing activities of daily living; knowledge and application of the principles of prevention of deformities, the normal range of motion, body mechanics, and body alignment.

Applicants must have an up-to-date LPN license issued in the state of Pennsylvania along with Professional Liability Insurance, a state criminal background check (If you lived outside the state of PA in the last 2 years we also require an FBI background check), an up-to-date physical exam and 2 step PPD test, an up-to-date CPR/First Aid Certification approved by the American Heart Association, and of course a resume.
