CASE MANAGER I – SW (MSW) – CARE MANAGEMENT

The Social Work Care Manager, based on the clinical assessment of the physicians, nurses, and ancillary staff, will coordinate appropriate post-acute services for the patient upon discharge. In addition, resource utilization and regulatory requirements are considered when coordinating the patients’ needs through the continuum of care. The Social Work Care Manager will also offer psychosocial support to patients and families for adjustment to illness, grief counseling and psychiatric stabilization. The Social Work Care Manager practices a work-style reflecting the mission, vision and values of the Care Management team, as well as the Organization.

Education, Training and Experience Care Manager I – (MSW):

Master’s Degree in Social Work (MSW).

Minimum 1(1) years MSW experience in an acute healthcare setting

Skills or Other Qualifications Required:

Must be able to master detailed and complex information regarding benefits and coordination of care.

Self-directed individual with ability to prioritize multiple tasks and meet deadlines, excellent attention to detail, and maintain a high level of work quality.

Demonstrated ability to work on a multidisciplinary team.

Demonstrated ability to function independently as a collaborative, supportive team member.

Must have solid psychosocial assessment skills.

Knowledge of chronic and acute disease and how it impacts patient and family functioning.

Proven ability to interface with all customer levels (physicians, Organizational staff, insurance companies, patients and family members).

Working knowledge of Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint, plus healthcare database systems.

Decisive and capable of exercising good judgment under pressure.

Preferred:

Qualified bilingual or multilingual candidates.
