The LPN is responsible for managing, delegating, coordinating and providing nursing care to patients at the practice, performing in a navigator role for the physician and must be able to work independently as needed. Must be an accomplished LPN with the utilization of effective communication skills with patients, physicians and members of the multidisciplinary health care team. Within the scope of practice assumes responsibility, as necessary, for the assessment, treatment and care of patients. Displays an understanding of the developmental needs of the patient when planning and implementing treatment. Displays competency in obtaining information and interpreting the patient’s behavior based upon an understanding of the patients cognitive, biopsychosocial and religious cultural aging processes. The LPN is accountable for maintaining his/her clinical competence in reaching the defined standards of care for the area of nursing practice.

NYS LPN License, Current NYS Registration, BLS Certified. Must have minimum 3 years of experience as an LPN and be able to work in the fast pace environment in a Family Practice. Must be able to administer vaccinations and other injections and have proficient work experience in phlebotomy. Must be very familiar with today’s medications and with the prior authorizations process. Have working knowledge of an EMR system.
