Job Description:

Clinical Liaisons support short term acute care hospitals in their local communities. Our liaison team consists of Nurses and Respiratory Therapists who collaborate with STACH personnel to identify patients who can benefit from long term acute care services. Armed with a strong clinical background, payer knowledge and communication skills, liaisons work with physicians, discharge planners, and patient families to plan the best continuum of care for critically ill patients who require a longer term of stay in an acute setting. This is a salaried position that works Monday- Friday with some on-call responsibility.

Responsibilities include:

  • Use your strong clinical knowledge, communications skills, and problem solving abilities to build productive relationships with physicians and discharge planners at hospitals.
  • Be results focused in order to meet census goals.
  • Meet with referring hospitals every day to help identify and transition patients who will benefit from LTACH services.
  • Perform pre-admission assessments on referred patients.
  • Educate patients, families, hospitals and healthcare professionals on the benefits of LTACH care.
  • Monitor marketing activity through call logs (activity logs) and profiling of customers is a necessary skill.
  • Qualified candidates must be passionate about providing a superior quality of care. You must also be a resourceful problem solver who thrives in a fast-paced environment.

    Clinical Liaisons must:

  • Be a licensed Nurse or Respiratory Therapist with acute care experience
  • Have a professional demeanor with excellent communications skills, preferably with sales, marketing and
  • Be flexible, self-driven, accountable for results, autonomous, yet supportive of team efforts.
  • Have valid driver’s license, proof of valid auto insurance, and active clinical license are required.
